Our Mission

At PENS, it is our mission to uphold the highest moral and ethical standards, while providing cutting-edge, evidence-based diagnostic and treatment modalities to ALL patients equally, with the main goal of improving their health and overall quality of life, in the most respectful and professional manner.

Appointments

PENS patients are seen by appointment only. You can now request an appointment by e-mailing us at [email protected] .

If you’re calling for a new patient appointment, you will need to have:

  • Child’s full name
  • Date of birth
  • Social security number
  • Primary care physician
  • Health insurance information
  • Current mailing address
  • Home phone number
  • Work or cell phone number

Arriving 10 to 15 minutes early is required to complete necessary paperwork, or you can easily and conveniently fill out all new patient forms on our website prior to your visit! Look under the Patient Forms tab. You should also expect to receive an e-mail from us with the forms attached.

New patients must be accompanied by their biological parent or legal guardian during their first visit. Foster care providers, as well as children in the custody of a non-biological parent, should bring proper documentation. Please bring a photo ID and insurance card.

Establishing Guardianship
Please provide one of the following forms if the patient will not be accompanied by a biological parent:

  • Birth certificate with parents’ names on it – for adopted children
  • Signed, dated letter giving permission to a designated person to accompany patient to their appointment and discuss patient care with the provider. A copy of parent ID must be included with the letter.
  • Official court guardianship papers
  • Hillsborough Kids, Inc. papers
  • Residential facilities authorization for treatment (group homes, juvenile detention centers, etc.)

Insurance and Referrals

We accept all PPO's, most HMO's, Medicaid and Medicaid third parties. Please contact your insurance provider to determine requirements for referral prior to your visit. HMO plans require a referral from your child’s primary care provider prior to your visit. Without proper documentation, your appointment could be cancelled or rescheduled.

Please bring your insurance card to your appointment. Please come to your appointment prepared to pay your co-pay, deductibles, co-insurance and/or any other patient responsibility amounts. These are due at the time services are rendered. We accept cash, checks and credit cards.

Testing

Our physicians may recommend that the patient have tests completed prior to the next visit. A list of facility locations where these tests can be performed will be provided to you at the time of the visit. If testing is recommended and your insurance requires authorization, our office will require notice of at least 5 business days prior to testing in order to obtain authorization. Please call our office and provide the date, time and place where the test will be done.
